Welcome To Albuquerque Surgical Consultants!

In order to make your first visit a pleasant one, there are several options to choose from in registering with us:

  • Download and print out a PDF (236 KB).
  • Call (505) 998-SURG or toll-free (877) 998-7874, and we will mail out the necessary forms.
  • Or, simply arrive at our office 10-15 minutes early the day of your appointment.

Please fill out these forms clearly and completely. If you are filling out the registration forms beforehand, please bring them in for your first visit and give them to our receptionist.

What Else To Bring:

  1. any relevant x-ray and medical records.
  2. your insurance card. We will make a copy and return it to you.
  3. your co-pay in the form of cash, check or Visa/MasterCard, or if you are a self-pay patient, please bring at least a partial payment.
  4. if required by your managed care program, a written referral from your primary care physician.

It is vital that you list all medications, dosages and drug allergies on the attached form.

Why Take the Time To Do This Before Your Appointment?

When you do this, it will be a more efficient use your time and allow you to see Dr. Morris with a shorter wait. Our respected and well-established surgeon will take care of you from there. If Dr. Morris determines that surgery is required, you will speak with a member of our staff to set a date and time for your surgery. Please know your availability so that we can make these arrangements during your visit.
